Emergency Card
Free emergency medical card for your phone: allergies, medications, conditions and contacts, with a QR code. Stays on your device and works offline.
Emergency Card keeps your critical medical details in one place on your phone, so you or a first responder can find them quickly. You enter your blood type, allergies, conditions, medications and emergency contacts, and the tool shows them as a clear card with a QR code. It is free and needs no account. Data never leaves your device - stored locally in your browser. Works completely offline once loaded. Install to your home screen for quick access in emergencies.
How to use it
- Fill in the form: name, date of birth, blood type, allergies, medical conditions, current medications (name, dose and frequency), emergency contacts (name, relationship and phone), notes for first responders such as a pacemaker or DNR status, and your organ donor preference. Everything saves as you type.
- Tap View Card. Allergies sit at the top of the card, and each contact's phone number is a tap-to-call link.
- When your browser offers Install, add the card to your home screen so it is one tap away.
- Use Export to download a JSON backup, and Import to restore it on another phone or browser.
What the QR code holds
The QR code on the card contains the fields you filled in as plain text: name, date of birth, blood type, allergies, conditions, medications, contacts, notes and donor status. Anyone who scans it can read that text without an internet connection, and nothing is looked up online. Show it only to people you want to have this information.
Questions
Where is my information stored? In your browser's local storage on this device. It is not uploaded or synced. Clearing your browser data deletes it, so keep an exported backup somewhere safe.
Does it replace a medical alert bracelet or my phone's built-in medical ID? No. Those can be read without unlocking your phone. Use this card alongside them, for the detail they do not have room for.
Is the card in other languages? The form and the card are in English.
